MicroPen EVO microneedling is a minimally invasive treatment designed to improve skin texture and support the natural production of collagen and elastin. The treatment uses a handheld device containing small, sterile needles to create precise microchannels within the skin.

These controlled micro-injuries activate the skin’s natural repair process. As new collagen develops, the treated skin may gradually appear smoother, firmer and more even.

How does MicroPen EVO work?

Collagen and elastin help give the skin its strength, firmness and elasticity. Natural production of these proteins decreases over time, contributing to wrinkles, enlarged-looking pores, uneven texture and other visible changes.

During treatment, the MicroPen EVO device moves rapidly across the skin while its sterile needles create microscopic channels at a customized depth. These channels trigger a healing response that supports new collagen-rich tissue and gradual skin remodeling.

MicroPen EVO does not use radiofrequency energy or laser heat. The treatment relies on mechanical microneedling to stimulate the skin.

Can microneedling treat melasma or hyperpigmentation?

Microneedling may be incorporated into a broader treatment plan for selected patients with uneven pigmentation or melasma. However, melasma is a complex condition that can worsen after inflammation or sun exposure.

Your provider will evaluate your natural skin tone, type of pigmentation, current medications and history of discoloration before recommending treatment. Microneedling may be combined with an appropriate at-home skin-care regimen, but results vary and recurrence is possible.

What areas can be treated?

Common treatment areas include the:

  • Face
  • Neck
  • Décolletage or chest
  • Abdomen
  • Other areas affected by selected scars or stretch marks

The treatment depth is customized because skin thickness and sensitivity vary between different areas. Your provider will determine whether microneedling is appropriate for the location and concern being treated.

What happens during treatment?

Your appointment begins with an evaluation of your skin, medical history, current products and treatment goals. The area is cleansed and a topical numbing medication is usually applied for approximately 30 to 45 minutes.

Once the skin is numb, the MicroPen EVO device is moved methodically across the treatment area. The needle depth may be adjusted throughout the procedure according to the thickness of the skin and the concern being addressed.

After treatment, the skin is gently cleansed and approved hydrating or protective products may be applied. The microneedling portion generally takes approximately 30 to 45 minutes, while the complete appointment typically lasts one to one-and-a-half hours.

Is microneedling uncomfortable?

Most patients tolerate MicroPen EVO treatment well after topical numbing medication has been applied.

You may feel:

  • Light pressure
  • Vibration
  • Prickling
  • Scratching
  • Warmth
  • Increased sensitivity over bony or thin-skinned areas

Areas such as the forehead may feel more sensitive than the cheeks. Comfort varies according to the treatment depth, location and individual sensitivity.

How many treatments will I need?

A series of three treatments spaced approximately four to six weeks apart is generally recommended for the best results. More noticeable acne scars, stretch marks, wrinkles or textural concerns may require additional sessions.

The number of treatments recommended will depend on:

  • The concern being addressed
  • Its severity and duration
  • The size of the treatment area
  • Your age and skin condition
  • Your natural healing response
  • Previous procedures
  • Your desired level of improvement

Your provider will evaluate your response before recommending additional or maintenance treatments.

When will I see results?

The skin may appear brighter and feel smoother after the initial redness and dryness resolve. These early changes are partly related to temporary swelling and surface renewal.

More significant improvements develop gradually as new collagen forms and the skin remodels. Changes in texture, scars, pores and fine lines may continue becoming more noticeable over the following weeks and months.

Because collagen production takes time, the final result cannot be evaluated immediately after treatment.

How long do the results last?

Collagen remodeling can provide long-lasting improvement, but microneedling does not stop aging, sun exposure or the development of future skin concerns.

After completing the initial series, maintenance treatment may be recommended anywhere from approximately every three to 18 months. The ideal schedule depends on your skin, the concern treated and how long your results remain noticeable.

Daily sunscreen and a consistent, professionally recommended skin-care routine can help preserve your results.

Is there any downtime?

Most patients experience approximately one to three days of visible redness or sensitivity. More intensive treatments may require additional recovery time.

Temporary effects may include:

  • Redness
  • Mild swelling
  • Warmth or tenderness
  • Tightness
  • Dryness or flaking
  • Pinpoint bleeding
  • Mild bruising
  • Itching
  • A rough or sandpaper-like texture

The treated skin may initially resemble a mild to moderate sunburn. Redness often improves within 12 to 48 hours, while dryness or flaking may continue for several days.

When can I wear makeup?

Avoid makeup for at least 24 hours after treatment or for the period recommended by your provider. Applying makeup too soon may irritate the skin or introduce bacteria into the temporary microchannels.

When makeup is resumed, use clean brushes, sponges and applicators. Stop using the product and contact the clinic if it causes significant burning, itching or irritation.

What enhancements are available?

Selected enhancements may be offered as part of a personalized treatment plan.

  • Platelet-rich plasma: PRP is prepared by drawing a small amount of your blood and processing it to concentrate the platelet-rich portion. The PRP may then be used with the microneedling procedure.
  • ZO® Brightalive Skin Brightener: This topical product may be recommended for patients concerned about uneven-looking skin tone, visible sun damage or discoloration. Your provider will explain when and how it should be applied.

The FDA has not cleared microneedling devices specifically for use in combination with PRP, medications, cosmetics, creams or serums. This means the safety and effectiveness of the combined use have not been evaluated as part of the device’s FDA clearance. Your provider should explain the proposed combination, expected benefits, alternatives and potential risks before treatment.

How should I prepare for treatment?

Before your MicroPen EVO appointment:

  • Avoid tanning and excessive sun exposure
  • Use broad-spectrum sunscreen daily
  • Avoid self-tanning products on the treatment area
  • Follow instructions about stopping retinoids and exfoliating products
  • Tell your provider about all medications and supplements you take
  • Report medications that affect bleeding or healing
  • Tell your provider if you have a bleeding disorder
  • Report any allergy or sensitivity to numbing medication or metal
  • Tell your provider if you are prone to cold sores
  • Report previous keloids or raised scars
  • Discuss recent fillers, botulinum toxin, lasers, peels or other procedures
  • Report active acne, rashes, infections, wounds or significant irritation
  • Arrive with clean skin and no makeup when possible

Topical retinoids are commonly stopped approximately one to three days before treatment, although your provider may recommend a longer interval based on the product and your skin.

Do not stop aspirin, anticoagulants or another prescribed medication unless the healthcare professional who prescribed it instructs you to do so.

What if I have a history of cold sores?

Microneedling around the mouth or face can trigger a recurrence of herpes simplex in patients who are prone to cold sores or fever blisters.

Tell your provider about your history before treatment. Preventive antiviral medication may be prescribed when appropriate. Treatment should be postponed if an active cold sore is present.

How should I care for my skin afterward?

Follow the personalized instructions provided by your treatment team. Freshly treated skin can be more sensitive to sunlight, heat, cosmetics and active ingredients.

You may be advised to:

  • Cleanse the skin gently
  • Use only approved moisturizers or healing products
  • Avoid makeup for at least 24 hours
  • Avoid picking, rubbing or scratching the skin
  • Avoid retinoids, exfoliating acids, scrubs and irritating products
  • Avoid strenuous exercise and excessive sweating temporarily
  • Avoid hot tubs, saunas, steam rooms and very hot showers
  • Limit direct sun exposure
  • Apply an approved broad-spectrum sunscreen with an SPF of 30 to 50
  • Allow dryness or flaking to resolve naturally

Some moisturizers, sunscreens or cosmetics may temporarily sting after microneedling. Contact the clinic if irritation is significant or continues to worsen.

What are the possible risks?

Temporary redness, swelling, tightness, peeling, bruising and pinpoint bleeding can occur after microneedling.

Less common complications may include:

  • Infection
  • Prolonged inflammation
  • Acne or cold-sore flare-ups
  • Temporary or lasting darkening of the skin
  • Temporary or lasting lightening of the skin
  • Persistent redness
  • Lines or track marks
  • Allergic or irritant reactions
  • Changes in skin texture
  • Scarring
  • An unsatisfactory or limited result

Using a sterile, single-use needle cartridge for every treatment and following proper aftercare are important for reducing infection risk.

Contact the clinic if you develop increasing pain, significant swelling, blistering, pus, spreading redness, fever or another unexpected reaction.

Can MicroPen EVO be used on different skin tones?

Microneedling does not depend on pigment-targeting light or laser energy, so it can often be used across a broad range of skin tones.

However, any procedure that causes inflammation can potentially trigger temporary or lasting discoloration. This risk may be higher in patients who are prone to post-inflammatory hyperpigmentation or who expose the treated skin to sunlight during recovery.

Appropriate needle depth, careful patient selection, gentle aftercare and consistent sun protection remain important for every skin tone.

Who is a good candidate for microneedling?

MicroPen EVO may be appropriate for adults who want to improve acne scars, fine lines, pores or uneven texture without undergoing surgery or a more aggressive resurfacing treatment.

Good candidates generally:

  • Have healthy, intact skin
  • Have realistic expectations
  • Understand that several treatments may be necessary
  • Can follow preparation and aftercare instructions
  • Are willing to protect the skin from sunlight
  • Do not have an active infection in the treatment area

An individual consultation is necessary to determine the safest treatment depth and whether microneedling is appropriate for your concern.

Who may need to postpone treatment?

Microneedling may need to be postponed or avoided if you:

  • Have active inflammatory or cystic acne
  • Have an active skin infection
  • Have an open wound, sore or irritated skin
  • Have an active cold sore
  • Have an eczema, dermatitis, psoriasis or rosacea flare
  • Have a history of keloids or abnormal scarring
  • Have a bleeding or clotting disorder
  • Take medication that significantly affects bleeding or healing
  • Have a weakened immune system
  • Have uncontrolled diabetes or another condition that affects healing
  • Are pregnant or breastfeeding
  • Are currently taking isotretinoin or recently completed treatment
  • Have an active or suspicious skin growth in the treatment area
  • Are allergic to stainless steel or the proposed numbing medication

Your provider will review the timing of isotretinoin and other medications individually rather than relying on a single waiting period for every patient.

Can microneedling be combined with other treatments?

MicroPen EVO may be incorporated into a broader treatment plan involving skin-care products, chemical peels, injectables, lasers or other aesthetic services.

These procedures are not always performed during the same appointment. Proper spacing may be needed to avoid excessive inflammation and allow the skin to heal.

Your provider will recommend the safest sequence based on your treatment goals, skin sensitivity, recent procedures and risk of discoloration.
